Welcome to the background screening website for Ascension. As part of the pre-employment screening program, we require that all candidates complete a background screening. This screening will be conducted by Universal Background Screening which maintains the highest level of security and confidentiality.

Before you begin, please be sure that you have the following information available:

• Social security number
• Birth date
• City, state and zip code of all your residences within the past seven years
• Driver’s license number and State where it was issued
• Education (Highest level of education such as High School/GED, Associates, Bachelors, Masters, or Doctorates)
• Employment (Two most recent employers or one employer over the past 7 years)
                  *If form is pre-populated with employers, please do NOT delete - but complete any missing/required information.  Employment verification is based on cumulative experience required for the Ascension role.      
                  *If Ascension is a previous employer, please do not list.  Please provide additional employer information.
