Before you begin, please be sure that you have the following information available: social security number, birth date, and the city, state and zip code of all your residences within the past seven years. Also, you will be required to enter a minimum of one education level for verification purposes. This should be the highest level of education you have obtained.

Current employees: Tomah Health is required by law to complete background checks on all employees every four years and driving record checks periodically on employees who regularly operate a company vehicle or use their own vehicle for company purposes. If you are receiving this notification, it is time to complete either a background check or a driving record check on you.
